What you'll be doing As the Clinical Coordinator you will provide care and advice for customers of all ages and cultural groups. You will engage with staff and customers to promote and facilitate independence to customers receiving our services and provide phone support to assess and develop strategies to support people requiring assistance to remain living independently within their own home. The role is responsible for educating and providing support and advice to our Support Workers, Service Support team and Client Service Officers on medications, clinical and general service delivery issues. In addition, the Clinical Coordinator manages and conducts credentialing training sessions for Support Workers. The role will promote Calvary Community Cares profile, clinical and non-clinical services and identify future clinical service delivery opportunities including hospital avoidance; early discharge and palliative care. The Clinical Coordinator will act as a relief to the Afterhours Clinical Care Coordinator during periods of leave and provide direct nursing care to clients when needed to ensure efficiency of service delivery. What you bring We are looking for Registered Nurses who have the following: APHRA registration and clinical experience; Knowledge of complex wounds, IV medication management, PICC lines , pumps and infusers; Ability to assess, monitor and evaluate clinical services provided to clients living with acute and chronic diseases; Ability to assess and develop innovative strategies to support people requiring assistance to remain living independently within their own homes; Proficiency in the use of Client Management Systems, Microsoft Office and Outlook; Well-developed oral and written communication skills. Experience in community nursing with understanding of the range of issues involved in the delivery of community based clinical care within the home setting would be an advantage.
