About the role

An opportunity exists for a suitably qualified Registered Nurse to join the Ambulatory Aged care Programs. This position is a fixed term part time position. The role provides case management / care coordination to clients in a community setting on Transition Care, Restorative Care and GEM in the Home programs. These 3 programs form the Ambulatory aged care Programs.

The Clinical Care Coordinator is an integral part of the multidisciplinary team. They will work in consultation with Physiotherapists, OT’s, Team Assistants, Speech pathologists, Dieticians, Geriatricians and Team leaders to implement best practice for AACP clients.

This position will provide the successful applicant with valuable Case Management/Care Coordination skills which is a sought after quality in community health programs.

The successful applicant will have high level interpersonal and communication skills and a demonstrated capacity to work successfully in a team environment. Must have an advanced level of clinical skills and be able to demonstrate excellent time management.

Drivers licence essential.

We are looking for:

  • Current Registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse, Psychologist, Physiotherapist or Occupational Therapist
  • Relevant Tertiary Qualifications in Nursing or one of the Allied Health disciplines and eligibility for professional body membership
  • Demonstrated ability to case manage clients and coordinate services within a care coordination model
  • Minimum of three years clinical experience and the ability to practice flexibility across acute, sub-acute aged care and community sectors
