The Nurse Consultant Disability/NDIS and Complex Care will provide clinical nursing/midwifery expertise for specified individual patients/clients and/or groups/populations, and/or provide clinical leadership for an area of practice and may work in a variety of clinical settings. Employees classified at this level are experts within their area and stream of practice and have a significant degree of autonomy and decision making.
This position is to maintain contemporary knowledge about hospital avoidance and discharge strategies and internal and external services to coordinate safe patient discharge, and prevent unnecessary CALHN presentations and admissions, provide clinical leadership and education within the organisation, and act as a CALHN hospital avoidance and supported discharge expert, inclusive of building effective working relationships with relevant Government and Non-Government Organisations and internal stakeholders to monitor and escalate barriers to discharge and avoidable admissions. This includes providing an expert consultative service to CALHN and external services on NDIS process, to support hospital avoidance and discharge strategies for consumers with confirmed or potential disability.
This position will develop, coordinate and contribute to the operationalisation of strategies and initiatives in line with CALHN priorities to address capacity management within the Integrated care model.
This position will contribute to the assessment/triage and coordination of deteriorating patients within the span of the position. This position will assist in the expansion and ongoing implementation of rapid assessment initiatives and pathways.
Must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
