Based in Mount Gambier, you will undertake comprehensive nursing assessments of frail aged and disabled clients referred to the Limestone Coast Aged Care Assessment Team (ACAT). Taking into consideration clients’ support and accommodation needs, as well as their family’s/carer’s needs, you will coordinate service planning, liaison and referrals. You will have delegated authority to approve clients for Commonwealth-funded residential care, residential respite care, community care or other Commonwealth-funded programs as they become available. Your role will service the Limestone Coast Region and operate within the Commonwealth Department of Health and Wellbeing – Aged Care Assessment Program Guidelines. You will ensure accurate and timely completion of documentation and act as a resource person based on your knowledge, experience and skills in aged care nursing. Working with and promoting a consumer-directed model of care, you will promote continuity and consistency of care in collaboration with the ACAT Coordinator and Limestone Coast ACAT Team.
You will be registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate. With demonstrated competence in aged care nursing, you will have the ability to address the health needs of people in various aged groups and cultural backgrounds. Effective communication, problem solving, conflict resolution and negotiation skills will be essential. As an experienced nurse, you will be capable of prioritising workloads and meeting set timelines whilst working under minimal supervision.
