Join our Specialist Dementia Care Team and support the delivery of outstanding care for people living with Behavioural and Psychological Symptoms of Dementia.
The Specialist Dementia Care Program (SDCP) is a federal initiative in partnership with the state or territory governments in supporting care of residents in approved residential aged care facilities, who provide specialist care to people living with BPSD and are unable to be supported in main- stream residential aged care.
The Specialist Dementia Nurse Consultant will work within a multi-disciplinary team to assist in achieving a holistic, biopsychosocial assessment of the person, understanding the reasons they are experiencing BSPD and working to establish nonpharmacological strategies to support an improve their symptoms and experience.
About the role:
Reporting to the Nursing Director, this role is integral the effective provision of clinical nursing expertise for patients living with complex health needs and who are living with Behavioural and Psychological Symptoms of Dementia (BPSD) and are part of the Specialist Dementia Care Program (SDCP). Duties for the role will include:
- Supporting teams to reduce/stabilise symptoms in order to assist people in transition back to mainstream residential Aged Care.
- Development of dementia support plans, in conjunction with multidisciplinary team, the person and their family.
- To provide specialist support and model best person centred, evidence based clinical care.
- Promoting resident engagement by developing and carrying out meaningful activities for residents and model / coach the approach for staff
- Assist with determining the appropriate care pathways for patients with Behavioural and Psychological Symptoms of Dementia
About you:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Holds, or working towards, a tertiary qualification in Gerontology or Dementia Studies
- Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills.
- Ability to work effectively within a multidisciplinary team.
- Ability to prioritise workload and meet set timelines.
