THE OPPORTUNITY

Be part of delivering a new way of supporting injured and ill Victorian’s transition between income support systems and into new experiences of life, health, community and work.

You will develop tailored support plans and work directly with those individuals to facilitate a successful transition and build their capacity to achieve their life goals

YOUR RESPONSIBILITIES

  • Develop and implement individual transition plans based on sound assessment of individual needs and circumstances.
  • Deliver and coordinate support to the individuals and their families with the aim to achieve improve capacity for personal wellbeing and achieve work or work readiness.
  • Maintain accurate and up to date case notes and reporting on client progress.
  • Work proactively with Advisory Group to ensure program remains on track and insights and learnings are capture

ABOUT YOU

  • Qualifications in Social work, Community Services, Rehabilitation Counselling, Nursing, Allied Health or equivale
  • Experience with and/or understanding of the principles of ‘strength-based’ and ‘person-centred’ service planning
  • An understanding of community-based welfare, health and employment sectors including Centrelink, Commonwealth Employment Services and Community Health
  • Attention to detail and high level of accuracy, particularly with record keeping and file noting

At EML we are continuing to hire nationally during COVID-19. As a business, we are currently onboarding all new staff virtually to accommodate current restrictions. Once these restrictions are lifted, we will migrate back to a work from office environment with new starters commencing within the office environment for their onboarding period of at least 4 weeks.
