About the Role:
The Northern Adelaide Local Health Network is seeking experienced Nurse Consultants to join the Supportive Weekend Interprofessional Flow Team. This is a unique opportunity to join a dynamic and collaborative team dedicated to delivering exceptional patient care.
The Supportive Weekend Interprofessional Flow Team (SWIFT) are integral in assisting NALHN to achieve maximum patient discharges, particularly over the weekend periods.
In this role you will be part of a Multi Disciplinary team who will identify patients who are ready for, or nearing discharge, to ensure all requirement for discharge are completed, with the goal of eliminating or reducing preventable delays to discharge across 7 days.
As a Nurse Consultant, you will play a pivotal role in supporting patient flow and ensuring seamless care transitions across the Northern Adelaide Local Health Network (NALHN). Your expertise and clinical acumen will be invaluable in facilitating effective communication, coordinating discharge planning, and optimising resource utilisation.
Key responsibilities of this role include:
- Collaborating with multidisciplinary teams to streamline patient flow and enhance care coordination
- Utilising your clinical expertise to support safe and efficient patient discharges
- Identifying and addressing potential barriers to timely patient transitions
- Contributing to continuous improvement initiatives and process optimisation
In this role you will be rostered to work Thursday – Sunday.
To be eligible for this position, you must:
- be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate, OR
- be registered or eligible for registration as a Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.

Our Local Health Network Sites and Services:
Lyell McEwin Hospital – The major hospital in the north and north-east for emergency care, complex and multi-day surgery, medicine, obstetrics, paediatrics and outpatient services.
Modbury Hospital – A general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.
