About the Role:
The Northern Adelaide Local Health Network is seeking a compassionate and skilled Clinical Nurse to join our Hospital Avoidance Team.
The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.
In this position, you will play a crucial role in providing high-quality care and support to patients in an outpatient setting, reducing the need for hospital admissions. As a Clinical Nurse, you will work closely with a multidisciplinary team to assess patients, develop care plans and deliver exceptional nursing care tailored to individual needs.
Your primary responsibilities will include conducting comprehensive assessments, implementing evidence-based interventions and collaborating with patients, families and other healthcare professionals to ensure seamless care coordination. You will also have the opportunity to educate patients and their caregivers on self-management strategies, promoting independence and improved quality of life.
To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
