As a Clinical Nurse you will provide nursing services in a variety of health service settings which has been consolidated by experience and/or further study, developing from a competent to a proficient practitioner. You will accept accountability for your own practice standards, activities delegated to others and the guidance and development of less experienced staff. In this role you will provide proficient, person centred, clinical nursing care and/or individual case management to patients/clients in a defined clinical area; monitor patient/client care plans to ensure appropriate care outcomes are achieved on a daily basis; and oversee the provision of nursing care within a team/unit. You will be required to, within pre-determined guidelines, and in a multi-disciplinary primary health care setting, assess clients, select and implement different therapeutic interventions, and/or support programs and evaluate progress.
The Clinical Nurse will work within the Northern Adelaide Geriatrics service as part of an Multidisciplinary team to provide a comprehensive service to older people living within the Northern Adelaide Local Health Network. The Clinical Nurse focus will specifically be, as part of the team, to assist older people to remain in their own home to live safely and supported. The Clinical Nurses role works across the acute and community teams assisting better flow through emergency departments and supported discharge.
To be eligible for this position, you will be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.
