- Salary: $79,998 – $101,120 p.a. – RN2C; OPD Liaison Nurse Full-Time Temporary up until 28 February 2023
The OPD Liaison Nurse will work closely with Clinical Coordinator, Team Manager and other senior staff to identify consumers who have met their care coordination goals within community mental health services and where there are barriers to transfer to alternative services in primary care. Depending on the consumer’s needs this may involve: ensuring outcomes from OPD appointments are followed up regarding medication needs; addressing consumer issues; medication education; liaison with GP and other services; utilising nursing expertise post discharge; implementing and assessing engagement with a plan of care and /or facilitating service linkages with other mental health or primary health care services. The OPD Liaison Nurse may also refer consumers for targeted interventions where short term interventions, assessment or education are required to enable seamless care.
Clinical Nurses provide nursing services in a variety of health service settings which has been consolidated by experience and/or further study with staff at this level developing from competent to proficient practitioners. You will accept accountability for your own practice standards, activities delegated to others and the guidance and development of less experienced staff. In this role you will continue to be predominantly clinical in nature; however you will be assigned appropriate portfolios. The allocation of portfolio responsibilities should be negotiated and be consistent with the career development plan as determined by your performance review/development plan.
To be eligible for this position you must be registered as a Nurse by the Nursing/Midwifery Board of Australia (NMBA) and hold a current practicing certificate. You must be enrolled in an approved mental health course or hold a qualification in mental health practice.
