Overview

The Riverland Home Care Package Program offers high-quality, home-based care, supporting individuals to remain in their own homes with tailored services that respect their personal preferences and goals. We’re looking for a Case Manager to join our team and help make a positive impact on the lives of our consumers.

In this role, you’ll work closely with consumers, their families, advocates, and carers to identify their care needs and goals, and develop personalised service plans that empower them to live independently at home. You’ll design flexible, strengths-based care solutions, ensuring each plan aligns with budget and risk parameters while focusing on the consumer’s unique requirements.

This is a multi-classification role, with options for applicants from various backgrounds, including Enrolled Nurses. Enrolled Nurses will work closely with the Registered Nurse/Midwife, providing person-centred care and adhering to all regulatory requirements. At this level, Enrolled Nurses may be at the novice or proficient stage, working under supervision but always accountable for their actions in providing nursing care.

Essential Criteria

For ENC/END Classification:

  • Must be enrolled, or eligible for enrolment, as a Nurse with the Nursing and Midwifery Board of Australia.
  • Must hold, or be eligible to hold, a current practising certificate.
  • Previous experience in providing nursing care in a healthcare setting, in line with appropriate standards of practice.

For OPS2 Classification:

  • A qualification in health, aged care or primary health care promotion at TAFE SA Certificate 3 level equivalent or above.
  • Experience in the delivery of innovative consumer directed care for individuals.
  • Significant experience working in a community based setting.
  • Experience working within a multidisciplinary team setting.
  • Experience working with people with complex needs.

General Requirements for All Applicants:

  • Experience in the use of personal computers and computerised client information systems.
